Confidentiality of patients records is maintained at all times and records will not be passed on to third parties without your express permission.

During your initial assessment you will be asked about your current and past medical history, including medication (tablets, creams, injections, inhalers etc.), operations, health conditions and diseases and these will be recorded as an aid to diagnosis and treatment.
